Application fee: $35 (includes an IMBA shirt).
Annual membership fee: $20 for every year the student takes classes. There is also an opportunity to waive this fee by participating in a fundraiser where every student and parent is required to participate.
(Initials) Permission is granted to Instituto Mazatlán Bellas Artes to use photographs and videos of students during classes or performances for publicity purposes.
(Initials) I have read and understood the information packet and I agree to follow the rules listed in it. I also agree to pay the fees stated on time. I understand that my/my child(ren)'s opportunity to participate in performance will be affected if rules are not followed and/or fees are not paid. I understand that performing is a privilege, not a right.

Liability Waiver and Acknowledgement of Risk

I understand and agree that in participating in any dance class, workshop, rehearsals and/or performance, there is a possibility of physical injury and/or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child(ren) during any of the Instituto Mazatlán Bellas Artes classes, workshops, rehearsals, performances or activities.
I also exempt, release and indemnify the Instituto Mazatlán Bellas Artes studio, its owners, agents, volunteers, assistants, employees, guest artists, faculty members and students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my child(ren), or property which may arise out of or in connection with participation in any classes, workshop, rehearsals, performances or activities conducted by Instituto Mazatlán Bellas Artes.
I further, hereby, voluntarily agree to waive my rights and that of my heirs and assigns to hold Instituto Mazatlán Bellas Artes, its owner, agents, volunteers, assistants, employees, guest artists, faculty members and students liable, for such damage, loss, injury and/or death. I understand that I should be aware of my/my child(ren)'s physical limitations and agree not to exceed them. If I am signing this waiver for my child(ren), I certify that I am the parent or legal guardian and have the right to waive these rights.
